Note: we require a Federal W-9 form and a Certificate of General Liability and Worker’s Compensation Insurance sent directly from your insurance company. Policy limits and requirements are listed on the Subcontractor Info Requirements Form. If your company doesn’t carry a Worker’s Compensation Insurance policy, the State of Maine requires that an Application for Predetermination (WCB-266) be submitted for approval. When you receive that approval letter, Phi Builders + Architects will require a copy for our files.
